Smoking decreases both thyroid secretion and thyroid hormone action in women with

hypothyroidism. Smoking does not cause hypothyroidism, but increases its severity.

The most dramatic effect of smoking on the thyroid is its association with Graves'

hyperthyroidism. The odds ratio for smoking among patients with Graves'

hyperthyroidism with and without opthalmopathy were 7.7 and 1.9, respectively, and

smokers had more severe eye disease.

Graves' disease is associated with an overactive thyroid gland and often a swelling of

muscles behind the eyes that makes them bulge out (as with the actor, the late Marty

Feldman). 81% of a survey population with Graves' disease were smokers, and the

incidence of the disease was 7.7 times higher in smokers. Of the 27 people with the

worst form of the disease, 26 were smokers.
