Perinatal mortality was 270 per 1000 (27%) for children of smoking women in Dacca,

Bangladesh. This was twice as high as for children of nonsmokers.

Mothers who smoke have a significantly increased risk for episodes of uterine

bleeding during pregnancy. After birth, infants of smokers also had more frequent

(relative risk 2.76) and longer episodes of obstructive sleep apnea, or periods of not

breathing during sleep.

Exposure to maternal smoking during pregnancy, even if only in the first trimester, is

associated with long term and potentially irreversible reduction in lung function.

Infant mortality increases from 15 per 1000 live births in white nonsmokers to 23 per

1000 for white women who smoke more than one pack of cigarettes per day. The

comparable rates for black women are 26.0 for nonsmokers and 39.9 for the second

group. The odds for a spontaneous abortion in one study were 46% higher for the first

10 cigarettes smoked per day and 61% greater for the first 20 cigarettes smoked per

day compared with no cigarettes during pregnancy. Smoking also accounts for 21%

to 39% of the incidence of low-birth-weight-babies. In terms of fertility, smokers have

3.4 times the risk of taking more than a year to conceive, and twice the risk of infertility

(not pregnant 5 years after ceasing contraceptive use).
